Sometimes acne is nothing more than a simple pimple, a bit of trapped pus consisting of sebum, dead skin cells and white blood cells. But it can be more severe. Knowing which type you have is key to a proper diagnosis and treatment plan.

Professionals recommending a course of treatment will rank acne by Grade.

Grade I includes milder, non-inflamed types such as blackheads, whiteheads or milia. Grade II involves a larger number of these, and frequently includes papules or pustules that are mildly inflamed.

Grade II Acne. Papules are small lesions, a change or break in the skin, that is smaller than 5 mm – about the width of a thin cigarette. They’re in the form of a bump that rises above the surface. A pustule, by contrast, is pus-filled – a mixture of bacteria, white blood cells and dead skin cells.

Grade III is more severe. The papules or pustules will be red, larger and more numerous. Grade IV is the most severe level, and includes nodules and cysts. The inflammation is usually wide spread and includes more than just the face.

Acne vulgaris is the common form of mild acne. Any of the Grade I forms will fall under this heading. They can usually be treated with over the counter medications.

In more severe form acne can develop into a type known as acne congoblata. The name may sound amusing, but the condition is anything but. It’s characterized by heavy inflammation and deep abscesses. The result is often scarring and other forms of serious skin damage.

Inflamed nodules will form around comedones – a general term for blackheads, whiteheads and milia. They often grow until they spontaneously discharge pus. While they do, they’re painful. When they do, they’re unsightly. Keliod-type scars are commonly left behind.

Acne Fulminans

Another type is known as acne fulminans. Here, the nodules will often ulcerate, leading to a painful and recurrent form of acne. The person afflicted may even develop a fever or aching joints. Treatment with corticosteroids or NSAIDs (non-steroidal anti-inflammatory drugs) is usually called for. But these treat symptoms, not the underlying cause.

In still more severe forms, a person may develop nodulocystic acne. Cysts form (red bumps produced by severe inflammation), but they’re of an unusual type. Sometimes they become numerous and close enough together to form a larger inflamed area. When they form small tunnels under the skin, infection can spread more readily. A treatment of antibiotics is called for, and isotretinoin (brand name: Accutane) is frequently employed.

Gram-negative folliculitis occurs when the follicles – the shaft in which hairs grow and sebum flows from the sebaceous glands – become infected. Bacteria grow inside and the body responds by dispatching white blood cells to fight the infection. The result is a deep eruption that calls for special treatment.

The type of bacteria that produces it may be resistant to normal antibiotics. In fact, the condition may have resulted from treatment of other types of acne treated with antibiotics.

Proper diagnosis of these forms of acne calls for a visit to a professional. A dermatologist’s care is warranted in these cases, as it may be in other forms of severe acne.

A rehashed story gets legs again. I remember years ago when I took Accutane, actually I took it twice, I did have some of the side effects of depression. As most people that take Accutane are teenagers with hormonal issues already this can be expected. The thing that is most important for any parent or loved one to watch for signs of depression in people taking Accutane.

Anyway this is a true side effect that was reported originally back in the 80′s.

Canadian scientists believe that taking Accutane, the most commonly used acne drug, is associated with a higher risk of depression.

According to a study published in the Journal of Clinical Psychiatry, isotretinoin, the active ingredient of Accutane, doubles the risk of depression.

CHU Sainte-Justine Research Centre scientists say while depression is a rare side effect of isotretinoin therapy, close monitoring of isotretinoin users is necessary due to the serious consequences of depression.

They concluded that psychiatric assessments of patients prior to and during isotretinoin therapy are crucial.

Accutane, a form of vitamin A (isotretinoin), reduces the amount of oil released by the skin and helps the skin renew itself more quickly. The drug is an effective choice in treating severe nodular acne.

There are dozens of prescription medications to treat moderate to severe acne. Some are topical (applied to the skin surface), others are taken as pills or injections. All require at least one visit to a professional dermatologist or physician. Fortunately, the majority fall into a few simple classes with pros and cons that are relatively easy to describe.

Accutane by prescription

Accutane is one of the oldest, and among the most effective, prescription medications for the treatment of moderate to severe acne. It is used on cystic and other advanced forms and has been used safely by millions for that purpose. But it does have some potential side effects and risks.

Pregnant women are generally advised to avoid taking isotretinoin. Several studies have suggested that it can produce birth defects. It’s a synthetic molecule, a retinoid similar to Vitamin A, available in pill form. As such, it can be abused. Normal dosage is once or twice per day for 4-5 months.

It works to reduce oil production, unclog pores and kill the bacteria that contribute to acne. It can also cause headache, excessive skin dryness, diarrhea and other problems. However, like all side effects, everyone is affected differently. That’s one of the major reasons it’s important to see a professional regularly during the course of treatment.

Oral Antibiotics by prescription

Acne is caused, in part, by a bacteria (Propionibacterium acnes). So, the disease can be combated, in part, by taking antibiotics. Like all powerful antibiotics some care is necessary when using these.

Tetracycline (or its derivatives, doxycycline and minocycline) is one of the most commonly used. Both forms have been in use for many years and can substantially reduce inflammatory acne. Normal dosage is 5-10 grams per day at first, though it’s reduced as treatment progresses. Treatments may take several months. It, too, though carries some risk. Some patients’ skin becomes sensitive to the sun during treatment.

Erythromycin is another common treatment for acne. It’s a broad spectrum antibiotic and can kill Propionibacterium acnes effectively. Like any powerful antibiotic, though, it can sometimes cause gastrointestinal irritation. That’s a major reason it’s a prescription drug.

Topical Treatments by prescription

Some topical antibiotics can attack the Propionibacterium acnes bacteria directly through the surface of the skin. They’re typically used to treat less severe cases of acne. They may even be used in cases of acne vulgaris, a mild form.

A number of retinoids are sometimes prescribed. They may be the same compound used in over the counter medications, but in dosages high enough to warrant or require a prescription. Adapalene is a gel or cream that is usually effective within two or three months. It can cause minor skin dryness and irritation. It helps unclog pores. Tretinoin is a natural retinoid that has similar effects.

Anyone who has tried over the counter medications or treatments and not received satisfactory results has many other options. Nearly all acne can be successfully treated. Get a professional diagnosis to explore your options.

Some acne cases require more advanced treatment methods. In some individuals, the motivation isn’t just the severity of the acne as much as the desire to avoid side effects from medication. Whatever the motive, advanced acne treatments offer an alternative that is attractive to many. Among the most popular are light and laser treatments.

In decades past UV was used as a light source. It has the beneficial effect of killing the Propionibacterium acnes bacteria that plays a role in acne formation. But, when studies showed that UV can increase the odds of skin cancer, the treatment became much less popular.

Other forms of light don’t have that problem.

Blue Light to treat acne

One form is called ‘blue light therapy’, where the name derives from the color of the light used. The main effect is to kill the bacteria, but there is some effect on the skin oil. It can cause a slight dryness. The narrow band light contains a negligible amount of UV wavelengths.

Patients receive treatments twice per week over a period of about a month. It’s effective on forms other than more severe acne, such as those that produce nodules or cysts. The latter can actually be made worse with blue light treatment. Improvements ranged from complete remission to about 40% decrease in the number of lesions, in several studies. About two-thirds saw between a 59% to 67% reduction of inflammatory acne lesions.

Light Heat Energy to treat acne

Other forms of light therapy, such as LHE, have similar effects – they kill acne bacteria – but do so by a different means. LHE also tends to decrease sebum (the natural skin oil). Excess sebum production is considered one of the major factors encouraging acne development. This green light tends to shrink sebaceous glands.

The therapy is approved by the FDA for treating mild to moderately severe acne. Both inflammatory and non-inflammatory acne can be reduced by two treatments per week over a period of one month. Drug-based treatments tend to take two or three months or longer.

In some cases, though, light treatments are combined with the use of prescription medications.

ALA to treat acne

ALA is a compound applied to the skin. It doesn’t have any of the antibacterial properties of standard acne treatments like Accutane. It works by making the skin more sensitive to light, thus increasing the effectiveness of light treatment.

It’s kept on for 15 minutes to an hour, during which the patient receives blue or red light treatment. The jury is still out so numbers are sketchy, but early results suggest the treatment is safe and effective.

Lasers to treat acne

Normal light contains waves that scatter in all directions. Laser light is called coherent because the waves all move in an organized train in the same direction. That’s one of the reasons lasers can do all the things they’re used for.

The chief difference, though, between different lasers (just as it is with other forms of light) is the wavelength. In the case of acne treatment lasers, one popular type produces waves of 1450 nm (nanometers, a billionth of a meter). This type is particularly safe and effective.

In one study, one treatment reduced acne lesions by nearly 40%. Two treatments decreased them by nearly 60% and with three the number rose to over 80%. There is some mild pain associated with the treatments, however, so a topical anesthetic is used during the procedure.

As prom season looms, here’s some advice from Dr. Jason Green, a dermatologist, on treating and preventing acne.

1) Hormones, stress: “No one’s too old to have acne,” Green says. “It’s not restricted to puberty.” Factors that can aggravate acne include an increase in male hormones in males and females, menstruation, emotional stress, hair products and oil and grease from cosmetics. Vigorous face-washing can also make acne worse, as well as astringents, which strip skin of natural moisturizers.

2) Stop squeezing: “The most important thing is to prevent scars,” Green says. “Do not pop, squeeze or pick at acne. Seek medical treatment early if it doesn’t respond to over-the-counter medications.” Several therapeutic regimens prevent future eruptions and therefore take about eight weeks to work. The first line of therapy is a retinol — retin A micro, Tazorac or Differin. As an added bonus, they also have FDA approval to treat fine lines and wrinkles. Salicylic acid and strong versions of benzoyl peroxide are good, too. A salicylic chemical peel is a quick way to improve current acne.

3) Scarring: Microdermabrasion, in which a machine scrapes away the surface of the skin, is good for mild scarring. For more severe scars, breakthroughs include the Fraxel laser, Fractional laser and the fillers Radiesse, Juvederm and Perlane. Accutane, an oral retinoid, “causes all the sebaceous glands to shrink down, but it’s the big guns we use only for severe scarring acne or acne that doesn’t respond to other treatments,” Green says. Pregnant women cannot take Accutane. Women must get a blood and urine pregnancy test as well as complete blood work done once a month when taking Accutane, and men must also have blood work done to check their liver enzymes and cholesterol levels. A link between Accutane and depression is still being debated.

4) Quick acne fixes: If you need a quick fix for a date, prom night or wedding, mix a crushed aspirin, which is an anti-inflammatory, with a bit of water and apply it to a pimple overnight to decrease redness. Visine “gets red out of your eyes and can also get redness out of your nasty pimple,” Green says. Just soak a swab and apply it for 15 seconds.

5) No sharing: “We always discourage patients from sharing medications because of potential side effects,” Green says. Medications recommended by a doctor are tailored to the individual. What works for one patient might not work for another.

I just ran into some interesting information on vitamins that are useful in the prevention and treatment of acne. I can not seem to find the original source of this research as it seems to have bee ncopied around to various sites around the internet.

In a double-blind trial, topical application of a 4% niacinamide gel twice daily for two months resulted in significant in improvement in people with acne.4 However, there is little reason to believe this vitamin would have similar actions if taken orally.

Several double-blind trials indicate that zinc supplements reduce the severity of acne.5 6 7 8 In one double-blind trial,9 though not in another,10 zinc was found to be as effective as oral antibiotic therapy. Doctors sometimes suggest that people with acne take 30 mg of zinc two or three times per day for a few months, then 30 mg per day thereafter. It often takes 12 weeks before any improvement is seen. Long-term zinc supplementation requires 1-2 mg of copper per day to prevent copper deficiency.

Large quantities of vitamin A-such as 300,000 IU per day for females and 400,000-500,000 IU per day for males-have been used successfully to treat severe acne.11 However, unlike the long-lasting benefits of the synthetic prescription version of vitamin A (isotretinoin as Accutane-), the acne typically returns several months after natural vitamin A is discontinued. In addition, the large amounts of vitamin A needed to control acne can be toxic and should be used only under careful medical supervision.

In a preliminary trial, people with acne were given 2.5 grams of pantothenic acid orally four times per day, for a total of 10 grams per day-a remarkably high amount.12 A cream containing 20% pantothenic acid was also applied topically four to six times per day. With moderate acne, near-complete relief was seen within two months, while severe conditions took at least six months to respond. Eventually, the intake of pantothenic acid was reduced to 1 to 5 grams per day-still a very high amount.

A preliminary report suggested that vitamin B6 at 50 mg per day may alleviate premenstrual flare-ups of acne experienced by some women.13 While no controlled research has evaluated this possibility, an older controlled trial of resistant adolescent acne found that 50-250 mg per day decreased skin oiliness and improved acne in 75% of the participants.14 However, another preliminary report suggested that vitamin B6 supplements might exacerbate acne.

Most dermatologists are quite happy to very quickly throw a prescription for Accutane or Tetracycline at you. How about more holistic natural methods to treat your acne? Eating is a great natural acne treatment.

Instead of immediately trying to treat your acne with pills or creams why not try your best to be more healthy in the way that you eat and see what your eating is going to do to help you.

Most dermatologists and the health industry as a whole do not seem to care much about the food you eat but think about your body and the way that your body reacts to food in other ways. If you eat greasy food how do you feel? Tired? If you drink too much coffee how do you feel? Shaky and distracted?

Forgetting what the skin doctor days for a moment why not take a serious look at your diet and get really serious about cleaning up your diet. Here are some things that you should definitely avoid.

Greasy Foods – Deep fried foods are the very worst thing that you can eat. I have always found that my skin will break out if I eat a lot of greasy food.

Sugar – Sweets are very bad as they start your body in unnaturally increasing your metabolism and we do not even want to talk about how this will affect your blood sugar.

Caffeine – caffeine stimulates hormone production. This is bad.

Other than the above foods it is important to remember that dairy products like milk and cheeses as well as refined and cooked carbohydrates like flour, bread, rice, and chips are acid-forming in the body.

If you really want to get rid of acne, you need to eat more alkaline-forming foods like fresh vegetables and fruit, as well as some nuts and seeds.

Eating really can be a great natural treatment for acne. Looking at this list of foods you can probably guess that the more natural a food that you eat and the closer that it is to the source (unrefined) the better it will be for you.

One more thing to remember in this system is that you really need to drink water. Water is going to have a much bigger effect on your then almost anything else. What water will do is clean out your body as well as regulating your sweat and metabolism.

Try this natural acne treatment using healthy eating and your will see that it will help you get rid of your acne better then any prescription can.

Treatment of severe acne is something that some of us need to deal with at one time or another. Severe acne is when you have lumpy acne that does not go away and is very red. When I was younger I took Accutane to get rid of my severe acne as well as injections and drainage to get rid of my severe acne.

Drainage and Surgical Excision

Some large cysts do not respond to medication and may require drainage and extraction. Drainage and extraction, or acne surgery as it is also called, should not be performed by patients. Dermatologists are trained in the proper technique and perform acne surgery under sterile conditions. Patient attempts to drain and extract comedones by squeezing or picking, can lead to infection, worsening of the acne and scarring.

Severe acne requires an aggressive treatment regimen and should be treated by a dermatologist.

Acne Injections

When an acne cyst becomes severely inflamed, there is a good chance it will rupture and scarring may result. To treat these severely inflamed cysts and prevent scarring, dermatologists may inject such cysts with a much-diluted corticosteroid. This lessens the inflammation
and promotes healing. An interlesional corticosteroid injection works by “melting” the cyst over a period of 3 to 5 days. As painful as this may sound I never found it to be bad at all, the needle is this and it is really good to get rid of the lumpy red zits.

Accutane

Accutane is a potent drug reserved for treating severe cystic acne and acne that has proven itself resistant to other medications. Accutane is a synthetic (man-made) retinoid (form of vitamin A) that comes in pill form. It is usually taken once or twice a day for 16 to 20 weeks.

Today, it is the most effective acne treatment available because it is the only acne treatment that works on all four factors that predispose a person to acne – excess oil production, clogged skin pores, P. acnes and inflammation. The remissions achieved with Accutane usually last for many months to many years. For many patients, only one course of Accutane therapy is needed.

While Accutane is the most effective acne treatment available, it cannot be prescribed to everyone due to a number of potential side effects, some serious. One of the most serious side effects is the potential to cause severe birth defects in a developing fetus. For this reason, the U.S. Food and Drug Administration (FDA) requires that women not be (or become) pregnant while taking Accutane. FDA regulations require women of childbearing age to take 2 pregnancy tests prior to beginning Accutane therapy and use 2 forms of birth control for 1 month before therapy begins, while taking the drug and for 1 full month after therapy. It is also important that women not breast feed during this time. Women who wish to become pregnant after taking Accutane should talk to their dermatologist and gynecologist about when it is safe to get pregnant after receiving Accutane therapy.

Other possible severe side effects that may occur while taking Accutane include:

* Severe pain in the chest or abdomen
* Trouble swallowing or painful swallowing
* Severe headache, blurred vision or dizziness
* Bone and joint pain
* Nausea or vomiting
* Diarrhea or rectal bleeding
* Depresion
* Dryness of the skin, eyes and nose
* Thinning hair

If any side effect occurs, the patient’s dermatologist or other healthcare practitioner should be contacted immediately because some of these side effects can lead to serious health problems.

While taking Accutane, patients are regularly monitored for side effects through follow-up visits. For most people, these side effects are tolerable and not a reason to discontinue therapy before remission is achieved. However, it is important to keep appointments for follow-up visits because monitoring can reveal conditions that a patient might not notice. For example, a patient may not realize a rapid increase in bad cholesterol that is detected through a blood test.

The decision to use Accutane should be made jointly by patient and dermatologist. When used with all due caution under close medical supervision, Accutane can resolve severe acne that has not responded to other therapy. It has proven especially effective in resolving cystic acne, a severe form of acne that usually does not respond to other therapies.

For a substantial number of patients, one course of Accutane therapy is all they will ever need. A small number of patients require more than one course of Accutane therapy to control severe acne.

It is important to take Accutane as prescribed even if the skin clears before all of the pills have been taken to prevent relapse. At the end of an effective course of Accutane therapy, all or most of the acne lesion will have cleared. Patients may notice residual erythematous (reddish) macules (flat spots) where acne lesions were present. These macules are not scars, and they will fade in 6 to 8 weeks.

After therapy, patients should continue to follow the Skin Care Guidelines for People with Acne.

Antibiotic pills
Oral antibiotics have been a mainstay of therapy for severe acne for many years. Like topical antibiotics, oral antibiotics work to reduce the P. acnes population (a contributing factor in acne), which, in turn, decreases inflammation. Treatment with oral antibiotics usually begins with a high dosage, which is reduced as the acne resolves. Over time, the P. acnes bacteria can become resistant to the antibiotic being used to treat it. When this happens, another antibiotic can be prescribed. Numerous studies support the effectiveness of the following broad-spectrum oral antibiotics that are used to treat acne in the United States: doxycline, erythromycin, minocycline and tetracycline. For information about each of these oral antibiotics, see Prescription Acne Medications.

Birth Control pills
Birth Control pills have been shown to effectively clear acne in women by suppressing the overactive sebaceous glands. Oral contraceptives can be used as long-term acne therapy; however, this medication should not be prescribed to women who smoke, have a blood-clotting disorder, are older than 35 or have a history of migraine headaches without the advice of a gynecologist.

Spironolactone, a synthetic steroid, may be used in combination with birth control pills to treat acne in adult females. Spironolactone inhibits androgen production. Side effects include irregular menstruation, breast tenderness, headache and fatigue

Feel like you’ve tried everything to get rid of your acne? If you haven’t tried a prescription retinol yet, don’t throw in the towel yet. You may be pleasantly surprised by how effective retinoids can be as an acne remedy.

Basically, retinol is a vitamin A derivative. Vitamin A is beneficial for your skin, your vision, and your bones too. But to understand how retinoids control acne, it helps to know a bit about how pimples form in the first place. It’s a combination of factors that begin when the skin cells lining the hair follicles die and begin to slough off. Sometimes these cells get sticky, clump together, and plug up your pores, thus giving rise to pimples. Retinoids put a stop to this chain of events by bringing these cells back into balance and preventing them from sticking together. The cells are then free to slough off normally. In short, retinoids keep your pores open.

Retinoids also have anti-aging properties, another bonus for adult acne sufferers. The vitamin A derivative increases the amount of collagen in your skin, a protein that keeps your skin youthful and elastic. You may notice your skin looks more smooth and vibrant after using retinoids for a while.

Unfortunately, retinoids also have negative side effects. They can dry and irritate your skin at first, and may even worsen acne for a short time. To avoid stressing out your skin, you should begin by applying a topical retinoid no more than once a week, and gradually increase the number of applications each week. In most cases the skin will get acclimated to the retinol cream and won’t be irritated by it. Applying moisturizer can also offset the drying effects. Retinoids can also make your skin more sensitive to the sun, so applying a sunscreen is a must.

There are several different types of retinoids, including Differen, Retin-A, and Tazorac. Although most are applied topically as a cream or ointment, there is a retinol that can be taken orally. Isotretinoin (more commonly known as Accutate) is the most effective retinoid treatment of all. In fact, for many patients it stops acne indefinitely. However, Accutane is infamous for its side effects. It can cause birth defects if women take this medication while pregnant. It can also dry out your lips and nose, and cause nosebleeds. In rare cases, it has been linked with joint soreness, liver complications, and suicidal thoughts. For these reasons, it’s wise to work closely with a physician when taking an oral retinoid such as Accutane.

If you’ve been struggling with acne and feel like you’ve tried everything, a retinol may be just what you need. Retinoids are especially effective when combined with other acne treatments, such as a benzoyl peroxide spot treatment. Ask your dermatologist if retinoids are right for you.

Medical treatment for acne vulgaris is often recommended by doctors and dermatologists for moderate to severe cases of acne. If you just have mild acne, the best treatment is usually home acne treatment such as washing with a mild soap and having a healthy diet. The diet should include plenty of fresh vegetables and fruit, and less processed and sugary foods.

Medical treatment for acne vulgaris can take several forms. The first stage is to try applying certain topical treatments. You can try each of these in turn for a month to six weeks. If the acne improves, continue. If not, move on to the next option.

1. Benzoyl Peroxide

This can dry up acne vulgaris for many people. Products containing benzoyl peroxide include Brevoxyl and Benzac.

2. Salicylic Acid

This is another option which many people find helps their acne. Products containing salicylic acid include Propa pH and Stridex.

Aspirin is a derivative of salicylic acid and that is why some people treat pimples at home with soluble aspirin dissolved in a few drops of water to make a paste. However, it is more convenient to buy a commercial acne treatment. It looks pretty weird walking around with white aspirin paste on your face!

If neither benzoyl peroxide nor salicylic acid works for you, you will need to have a prescription medication.

3. Topical Antibiotics

The first step for a medical practitioner is often to prescribe antibiotic creams, gels or lotions which are applied directly to the pimples. These are for external use only.

4. Oral Antibiotics

In some cases you may be prescribed a course of oral antibiotics that you take internally. This treatment often helps acne scars to heal so it is more likely to be used where the acne is deep or is producing scarring.

If you take oral antibiotics, be sure to follow up with a course of probiotics afterward. You can buy probiotics from any health food store. This repopulates the healthy bacteria in the stomach. These can be killed off by antibiotics and the result can be an overgrowth of candida, leading to yeast infections.

If you think that you already have an overgrowth of candida, it is better not to take antibiotics for your acne but treat the candidiasis first. You may find that acne improves or even disappears when the yeast is under control. Candidiasis is not so likely to be a problem for teenagers, but is sometimes involved in adult acne.

5. Prescription Retinoids

Retinoids are related to vitamin A and they have many applications in medicine, one of which is treating acne. As with antibiotics, you can get topical retinoids in the form of creams or gels which are applied directly to pimples (e.g. Retin-A, Differin, Tazorac). Retinoids can irritate the skin so in some cases the acne gets worse before it gets better.

There are also oral retinoids that you take internally, such as Accutane. These usually take several months to have an effect. Oral retinoids should only be taken under medical supervision. There can be side effects and overdose is dangerous. Women taking retinoids must not get pregnant because they can cause birth defects, so oral retinoids are more likely to be prescribed for men as a medical treatment for acne vulgaris.
